We need your help. Finding effective solutions to improve health outcomes will take partnership and collaboration from all of us. We believe that by working hand in hand with the community we can save lives by redefining how, when and where we deliver care to best serve South Side residents.
Let's change the facts in our community
People living on the South Side of Chicago collectively have worse health outcomes than those living in other parts of the city. For example:
30-year gap in life expectancy
South Side residents live shorter lives than North Side residents – the largest gap in the country.
4X uncontrolled diabetes
Residents of South Side neighborhoods have a significantly higher rate of diabetes-related deaths compared to North Side residents.
Nearly 85% of hospitalized South Side residents have two or more chronic conditions such as:
- Hypertension
- Chronic obstructive pulmonary disease (COPD)
- Diabetes
- Obesity
- Congestive heart failure
- Mental health & substance use issues
- Renal failure
